Hairy Cell / Plasma Cell Leukemia Profile

 

 

Test Includes:

CD4, CD5, CD7, CD8, CD11c, CD19, CD22, CD25, CD38, HLA-DR; supplemental markers added as needed (CD103, CD138, CD71, CD2, CD56).

Specimen:

Whole blood or bone marrow aspirate.

Volume:

Two 7-mL EDTA whole blood tubes or one 7-mL EDTA whole blood tube and 2 cc EDTA or heparinized bone marrow aspirate. One EDTA whole blood tube is for an optional CBC.

Container:

Lavender-stopper (EDTA whole blood or bone marrow aspirate) tube or green-stopper (heparinized bone marrow aspirate) tube.

Collection:

Submit specimen at room temperature using Leukemia/Lymphoma Specimen Transport Kit (supplied by the Hospital). Draw specimen so it will arrive in the laboratory Monday through Saturday and within 24 hours of collection. Please state the date and time of collection on the test request form.

Storage:

Maintain specimen at room temperature.

Causes for Rejection:

Specimen frozen; specimen in formalin or another fixative; clotted specimen; hemolysis; blood more than 72 hours old; bone marrow aspirates older than 5 days.

Use:

This immunophenotyping profile is useful in the diagnosis and monitoring of hairy cell leukemia and the diagnosis of plasma cell leukemia.

Limitations:

This profile is not recommended for quantitation of plasma cell in the bone marrow.

Methodology:

Immunophenotyping by flow cytometry.

Additional Information:

Detection of a B-cell population coexpressing CD22, CD11c, and CD25 is useful in establishing a diagnosis of hairy cell leukemia when used in conjunction with morphology and cytochemistry. Immunophenotyping by flow cytometry is a sensitive method for detecting residual or recurrent disease in the peripheral blood of patients with an established diagnosis.

Detection of a population of cells expressing CD38 and CD138 in the peripheral blood is useful in establishing a diagnosis of plasma cell leukemia when used in conjunction with morphology.
